Peri-implant soft tissue management – strategies for biological and aesthetic long-term success

Clinical concepts in periodontal plastic surgery

Theoretical background:
Predictable coverage of multiple adjacent recessions and of single mandibular deep recessions is still a challenge for the clinician. Emerging evidence indicates that the use of the Modified Coronally Advanced Tunnel (MCAT) or the newly developed Laterally Closed Tunnel (LCT) have been shown to yield predictable coverage of single and multiple adjacent gingival recessions providing also long-term stability. Moreover, in certain, well selected cases these techniques can also be successfully applied to correct soft tissue defects at dental implants and alveolar ridges. Very recent data have also indicated that a newly developed collagen soft tissue replacement graft (Geistlich Fibro-Gide®) may be successfully used in conjunction with a tunnel approach for soft tissue augmentation at dental implants and alveolar ridges. The lecture will provide a comprehensive treatment philosophy and biologic principles that need to be considered to optimize the results. Presentations of clinical cases and of surgical videos will demonstrate the step-by- step procedure for the MCAT and LMDT in the treatment of various types of soft tissue defects at teeth, alveolar ridges and dental implants.

Complex vertical ridge augmentation with Yxoss CBR®

Theoretical background:
Complex vertical ridge augmentation frequently requires an intervention based on bone blocks from jaw angle or iliac crest. However, these procedures are difficult and time consuming. A new solution is based on an individual mesh for the patient, which fits exactly where there’s a bone need. Individual 3-D printed titanium scaffolds combined with biomaterials and autologous bone provide the advantage to plan the graft position precisely according to the subsequent prosthetic restauration. This workshop will show you how to move planning away from the operating room, with a short and extremely precise surgery.

Geistlich Fibro-Gide® - The alternative CTG for increasing soft tissue thickness
Theoretical background:
Soft tissue grafting procedures are considered an integral part of implant therapy and various techniques and materials are applied at different time-points. Whereas autogenous transplants are still widely used, they are associated with major disadvantages, most prominently the associated morbidity. Therefore, more recently, collagen-based soft tissue substitutes have been developed and demonstrate favorable clinical success and less morbidity. This workshop will focus on the clinical decision tree and include step-by-step procedures for the collagen-based matrice based on recent clinical and scientific evidence.

• Simuation of immediate implant combining hard and soft tissue augmentation
• Soft tissue thickening under pontics with classic flap
• Soft tissue thickening at implant sites with palatal island flap

Treatment of gingival recession with coronally advanced flap and Geistlich Fibro-Gide® as the alternative CTG

Theoretical background:
Single and multiple gingival recession are both unpleasant and unesthetic. Meeting the functional and esthetic demands of patients with gingival recessions defects remains a major therapeutic challenge. Therefore finding innovative therapeutic approaches - specially reducing patient morbidity has become a priority.
Hands-on parts – techniques presented:
State of the art volume-stable collagen matrix Geistlich Fibro-Gide® can provide an alternative and innovative treatment to gingival recessions. Step-by-step surgical procedure of the CAF with the adjunctive use of Geistlich Fibro-Gide® for the treatment of single and multiple gingival recession defects will be presented.

Successful concepts for challenging hard and soft tissue defects
Theoretical background:
Allogenic block grafts are being successfully used for 3D reconstruction in a variety of different bone defects. Key to success is meticulous soft tissue management, starting at planning of the incision line, mobilization but also subsequent soft tissue modification using collagen matrices. Technical and clinical considerations based on routine clinical use will be presented for bone grafting using the customized maxgraft® bonebuilder as well as soft tisse augmentation with mucoderm®.
Hands-on parts – techniques presented:
During the hands-on part flap preparation and suturing techniques will be demonstrated that enable tension-free soft tissue coverage after bone block augmentation.Vertical soft-tissue augmentation around implants as well as vestibuloplasty will be further adressed.

Novel regenerative concepts using dermal matrices – from integration to regeneration
Theoretical background:
New soft tissue grafting techniques will be compared with the gold standard – CTG, focusing on procedure-based and patient-based outcomes. Data illustrating key differences in available products will be presented to aid in product selection. Finally, tips for using the tunnel technique for minimally invasive, patient-friendly soft tissue grafting will be discussed.
Hands-on parts – techniques presented:
Using special tunneling instruments, perform the Tunnel Technique step by step: intrasulcular incisions, blunt dissection, sharp dissection, elevation of papillae interdentally, graft insertion, graft alignment, subpapillary continuous sling suturing method on a multiple tooth recession site.

The symbiosis of biology and clinical innovation – regenerative approaches in esthetically demanding situations
Theoretical background:
Implant therapy has become part of a dentist’s daily work. However, in many cases, the predictability of function and esthetics of an implant-supported restoration is questionable without supportive measures like bone grafting or Guided Bone Regeneration. The outcomes of these therapies are related to the surgeon’s skills and to the products and material used.
Hands-on parts – techniques presented:
The workshop features demonstration of relevant techniques for soft and hard tissue management, as well as introduction of bone grafting materials and membranes based on clinical cases. The hands-on section highlights soft and hard tissue management techniques like sinus lift and Guided Bone Regeneration, with demonstration and training on synthetic models.

State-of-the-art soft tissue management for aesthetic treatments
Theoretical background:
Discussion of the current state-of-the-art in socket preservation, bone augmentation and immediate implant placement in the anterior zone.
The workshop will mainly focus on peri-implant tissue management around anterior implants, using soft-tissue augmentation and provisional restorations. Current alternatives for the use of soft-tissue graft substitutes will be considered. Management of complications with implants in the esthetic zone will also be discussed.
